Our client is seeking a Patient Care Coordinator who will be directly coordinating communications with staff nurses, clinical pharmacists and dieticians.
The ideal candidate will have 2 years of customer service experience and previous experience working in the medical field.

Full-time, Monday to Friday, 8:00am-5:00pmLocation
Airport Area, Honolulu, OahuPay
$20 to $22 an hourResponsibilities
- Ensure smooth transition of care for each patient.
- Coordinate with home agencies and doctor offices to manage the continuity of care for therapies provided.
- Review clinical documentation and consult with a clinician to complete Medical Necessity.
- Gather and organize patient information.
- Coordinate nursing services with nursing agencies on Oahu, Hawaii, Maui, Molokai, & Lanai.
- Receive calls on nursing line and able to triage calls to clinician, patient care assistants or other departments.
- Assist with overall schedule maintenance of patients and nurses
- Perform other duties as assigned.
Qualifications
- Minimum 2 years of customer service experience.
- Previous experience working with other disciplines in the medical field (nurses, case managers, pharmacists, physicians, etc.).
- Strong communication (verbal, written, interpersonal) skills.
- Strong leadership skills.
- Ability to work individually and in a team setting.
