Our client is seeking a Patient Care Coordinator who will be directly coordinating communications with staff nurses, clinical pharmacists and dieticians.

The ideal candidate will have 2 years of customer service experience and previous experience working in the medical field.

Full-time, Monday to Friday, 8:00am-5:00pm

Airport Area, Honolulu, Oahu

Pay

$20 to $22 an hour

Responsibilities

  • Ensure smooth transition of care for each patient.
  • Coordinate with home agencies and doctor offices to manage the continuity of care for therapies provided.
  • Review clinical documentation and consult with a clinician to complete Medical Necessity.
  • Gather and organize patient information.
  • Coordinate nursing services with nursing agencies on Oahu, Hawaii, Maui, Molokai, & Lanai.
  • Receive calls on nursing line and able to triage calls to clinician, patient care assistants or other departments.
  • Assist with overall schedule maintenance of patients and nurses
  • Perform other duties as assigned.

Qualifications

  • Minimum 2 years of customer service experience.
  • Previous experience working with other disciplines in the medical field (nurses, case managers, pharmacists, physicians, etc.).
  • Strong communication (verbal, written, interpersonal) skills.
  • Strong leadership skills. 
  • Ability to work individually and in a team setting.
